Prior to your admission, as part of the pre-admission process, you will be asked to sign an "Assignment of Benefits" form which will be filed after your surgical procedure and will allow payment to be sent to the Yuma Surgery Center.
It is your right as a patient to request information or have questions answered regarding fees or payment policies. If you have any questions regarding your bill, you may contact our Finance Department between the hours of 7:30 a.m. and 4:30 p.m. Monday to Friday at 928-329-5011
